Great Basin Orthopaedic has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

How? Requests may be submitted electronically to HealthMark’s Request Manager at https://requestmanager.healthmark-group.com.

Once logged in, select “Submit Request” from the menu options and enter all required fields to provide an authorization directly to HealthMark.

Your medical record request will be processed and a notification will be sent via mail or email once complete and available for download.

The elbow outcome was excellent using GBO's Physical Therapy services and I am hopeful the hip will be better than new in a short while. I love how smoothly everything runs here. My doctor is amazing the nurse is great too. From checking in, to follow ups to my surgery (ACL) everything went so smoothly and even my post op care. I wish all my doctors outside ortho would go so smoothly! Great job guys!

I can not thank Dr. Dickens enough!!!! He got my daughter in the following Monday after her injury and then surgery the next day!!! So great full and thankful for him! He did awesome job as well with the surgery! Hardly don't see the scar!
